Register a dedicated-curve launch

For a token and curve you deployed yourself (not via the default /create flow) — once the Factory owner has run initialize_curve_for_deployed_token on your curve_id (see /admin), submit this form to register the launch yourself. Nothing is deployed here — this only calls create_launch.
